Professional Development Forms
To Apply For Funding:
The Professional Development Committee is excited to provide funding towards your professional growth. We offer two distinct options to support your development. Please remember that all funds are provided through reimbursement.
1. Professional Development Funds: Crafton Hills College faculty and staff may apply for funding support from the CHC Professional Development Committee for participating in training or attending conferences.
- Employees may request funding not to exceed $500 per year.
- Priority is given to conferences and training which benefit the college as well as the individual.
2. Equity Funds: The Professional Development committee would like to encourage professional development activities which focus on equity. Conference and activities supported by these funds must meet the goals of the Student Equity and Achievement Plan (SEAP).
- Employees may request funding not to exceed $500 per year.
- Equity Funds can be combined with Professional Development Funds, for a total of up to $1000.
3. Required Documents: Please complete, sign, and submit the documents requested below. All forms must be received before your request is presented to the Professional Development Committee for approval.
- Professional Development Conference Funding Request (online)
- Manager approved and signed SBCCD Travel/Conference Form (pdf)
- Conference agenda/literature (pdf only, hyperlinks to websites will not be accepted)
- Google map and number of miles traveling if you are requesting mileage (print outs must be provided, cell phone screen shots will not be accepted)
4. For additional request outside of conferences, seminars, and workshops, contact Kashaunda Harris, at kharris@craftonhills.edu to be added to the Professional Development Committee meeting agenda.
Submit all pdf documents to Professional Development at chcprofessionaldevelopment@craftonhills.edu. Your completed funding request documents will be taken to the next scheduled Professional Development meeting and voted on for approval by the committee.
*Receipts and any remaining documents must be submitted within two weeks of your approved conference or training to receive a reimbursement.
