If this is a sponsored event, Please fill out the on-line Sponsor Event form, print and have a contract employee sign it before returning it to the On Campus Facilities Use Office in LADM-167. A Contract Employee must be designated for any Sponsored Event on campus, soliciting contributions, inviting the public, or charging a fee. Please contact Business Services to answer insurance questions for events.

Facility Desired
List date or dates desired. The facility will be opened 30 minutes before event is scheduled to start and closed 30 minutes after the scheduled end time.

Maintenance Please note: It is not necessary to request Air/Heating/Power during normal school hours, M-F.
